Description

  • Certain work-related incidents require timely notification to MIOSHA:
  • Fatalities. Within 8 hours after the death of any employee from a work-related incident, you must report the fatality by telephone to the MIOSHA toll-free central telephone number: 1-800-858-0397.
  • Hospitalizations, amputations, and losses of an eye. Within 24 hours after the inpatient hospitalization of 1 or more employees or an employee’s amputation or an employee’s loss of an eye, as a result of a work-related incident, you must report the inpatient hospitalization, amputation, or loss of an eye to MIOSHA.
  • You must report the inpatient hospitalization, amputation, or loss of an eye using 1 of the following methods:
  1. By telephone or in person to the MIOSHA office that is nearest to the site of the incident.
  2. By telephone to the MIOSHA toll-free central telephone number: 1-844-4640-6742.
  3. By electronic submission using the reporting application located on MIOSHA’s web site at .

When an authorized government representative asks for the records you keep under these rules, you must provide copies of the records within 4 business hours.

RECORDING AND REPORTING OF OCCUPATIONAL INJURIES AND ILLNESSES. The issuance can be updated and superseded at any time by LARA. Note that the MIOSHA rules are more restrictive than OSHA at 29 C.F.R. Part 1904. Therefore, we follow MIOSHA at PA 154 of 1974 and all it's implementing rules. This section of rules appear at R 408.22001-22158.
