UPHS-Marquette Orientation Packet 

The following must be completed:

• Background Check (completed within 30 days of start date)
• Immunization Tracking Package (can be purchased anytime, but must be completed before start date)
• Requirements Information and Checklist
• Drug Screening (must be completed within 30 days of start date, at the location below)

  Clinic hours are routinely held 2 days each week on Tuesday and Friday between 8:30-5:00.  Appointments can be scheduled by phoning 906-475-7844.
