20 U.S.C. § 1087bb and 34 C.F.R. § 674.19: Fiscal Operations Report and Application to Participate (FISAP) reports cover the institutions fiscal operations and request for future funding for Title IV Federal campus-based funds (SEOG, Perkins Loans, Work-study, and Federal Pell Grants. Due by October 1 each year. The University must reconcile reported student earnings to complete Department of Education Fiscal Operations Report (Form 646-1) at the end of the academic year. REPORT DUE OCTOBER 1. REQUIREMENTS THAT GO ALONG WITH REPORT ARE LISTED IN INTERNAL NOTES.
