To appeal your academic suspension or dismissal from Northern Michigan University, this form and all official documentation must be submitted in order for the Admissions and Academic Policies Committee (AAPC) to hear your appeal and reach a decision. Illness – Letter from medical professional identifying patient, explaining medical condition, stating dates of illness and describing course of treatment
Death – Copy of obituary or death certificate
