Forms to Return

Requirements to Complete
Your DISA Healthcare Technology (CastleBranch) package must be purchased between Wednesday, July 29, 2026 and Wednesday, August 19, 2026. The following requirements must be completed by Wednesday, August 19, 2026, at midnight.
- Background Check and Medical Documents Instructions and Checklist
-Options for Marquette area CPR training through the American Heart Association - Basic Life Support (BLS)/BLS Provider/Heartcode BLS

Local Immunization Location Options

Clinic hours are routinely held 2 days each week on Tuesday and Friday between 8:30-5:00. Appointments can be scheduled by phoning 906-475-7844. - NMU Health Center - All clinic and lab visits are by appointment. Please call 906-227-2355.
