Incident Reporting Policy for Accidents, Injuries and Illnesses

Purpose

To establish a uniform procedure and format for reporting all accidents, injuries and illnesses, and near misses that occur on property owned or under the control of Northern Michigan University.

Policy

Public Safety must be immediately notified of all personal injury accidents* involving faculty, staff, students, or visitors. The injured individual must notify his or her supervisor, responsible faculty member, or NMU Police Department. A University Incident Report must be prepared within twenty-four (24) hours of the injury.

*A personal injury accident is an accident other than a motor vehicle accident. Motor vehicle accidents involving injuries are required to be reported by the Motor Vehicle Code.

Procedure for Reporting an Incident

NMU Employees and Students access the Incident Report Form through their MyNMU Portal, and clicking on the Safety Department tile or by clicking on the following link: https://apps.nmu.edu/incidentReport/

Visitors, rentals and those participating in university functions who use NMU facilities, report your incident to a staff member of the facility you are in or contact the NMU Police Dept at 906-227-2151. You may also email Safety@nmu.edu within 24 hours with the following information:

  1. Your Name,  Address,  Phone Number
  2. Time, Date and exact location of the incident (building & room/area)
  3. A brief description of the incident

Procedure for Reporting a Near Miss

A near miss is defined as an unplanned event that did not result in injury, illness, or damage, but had the potential to do so. Only a fortunate break in the chain of events prevented an injury, fatality, or damage from occurring.
