IMPORTANT HEALTH COVERAGE TAX DOCUMENTS
Notice Regarding Your Health Insurance Coverage Information (Form 1095-C):
Due to new federal legislation designed to reduce paperwork and administrative burden, we are no longer required to automatically provide you with a paper copy of your Form 1095-C each year. Form 1095-C is available to all active employees electronically via MyNMU, and available via paper to all current and non-employees/covered individuals upon request.
Form 1095-C contains details about the health coverage offered to you in the previous tax year. You do not need to file this form with your tax return.
How to Access Your Form 1095-C Electronically
Please use the following instructions if you wish to give your consent to receive Form 1095-C statements in electronic format after you have reviewed the Electronic Regulatory Disclosure Statement:
- Log into MyNMU
- Click on the Employee Self Service Tile
- Click on “Taxes” on the Employee Dashboard
- Click on “1095-C Employer-Provided Health Insurance Offer and Coverage Statement”
- NOTE: If you have not already provided electronic consent to access your 1095-C electronically you will receive a pop-up prompting you to do so prior to being able to complete the steps below.
- Select the appropriate year in the “Tax Year” drop-down box
- Click “Display”
- Click “Printable 1095-C” to generate a printable view of the Form 1095-C
- Print the Form 1095-C using the print feature on your web browser (ctrl + p)

How to Request a Paper Copy of Your Form 1095-C:
To get a copy of your Form 1095-C, you can use one of the following contact methods:

Upon receiving your request, your Form 1095-C will be provided within 30 days.
