Instructions to Provide Consent
Please use the following instructions if you wish to give your consent to receive all future regulatory statements in electronic format only and forego the paper copy after you have reviewed the Electronic Regulatory Disclosure Statement:
- Log into MyNMU
- Click on the Employee Services Tab (or Student Services Tab for Student Employees)
- Click on “Tax Information” under “Web for Employees” (or "Main Menu", then "Employee", then "Tax Forms" for student employees)
- Click on “Electronic Regulatory Consent”
- You will be directed to login again using your NMU username and password
- Scroll to the tax section of the employee dashboard
- Click on "Electronic Regulatory Consent" again
- Check the box next to “Consent to receive W-2 electronically” and/or “Consent to receive 1095-C electronically”
- Click “Submit”
Instructions to Withdraw Consent
Please use the following instructions if you wish to withdraw your consent to receive future regulatory statements in paper format only. You will receive paper regulatory statements for all future periods.
- Log into MyNMU
- Click on the Employee Services Tab (or Student Services Tab for Student Employees)
- Click on “Tax Information” under “Web for Employees” (or "Main Menu", then "Employee", then "Tax Forms" for student employees)
- Click on “Electronic Regulatory Consent”
- You will be directed to login again using your NMU username and password
- Scroll to the tax section of the employee dashboard
- Click on "Electronic Regulatory Consent" again
- Uncheck the box next to “Consent to receive W-2 electronically” and/or “Consent to receive 1095-C electronically”
- Click “Submit”
Instructions to access and print your Form W-2
- Log into MyNMU
- Click on the Employee Services Tab (or Student Services Tab for Student Employees)
- Click on “Tax Information” under “Web for Employees” (or "Main Menu", then "Employee", then "Tax Forms" for student employees)
- Click on “W-2 Wage and Tax Statement”
- You will be directed to login again using your NMU username and password
- Scroll to the tax section of the employee dashboard
- Click on "W-2 Wage and Tax Statement" again
- Select the appropriate year in the “Tax Year” drop-down box
- Click “Display”
- Click “Printable W-2” to generate a printable view of the Form W-2
- Print the Form W-2 using the print feature on your web browser (ctrl + p)
Instructions to access and print your Form 1095-C (not applicable for part-time employees)
- Log into MyNMU
- Click on the Employee Services Tab
- Click on “Tax Information” under “Web for Employees”
- Click on “1095-C Employer-Provided Health Insurance Offer and Coverage Statement”
- You will be directed to login again using your NMU username and password
- Scroll to the tax section of the employee dashboard
- Click on "1095-C Employer-Provided Health Insurance Offer and Coverage Statement" again
- Select the appropriate year in the “Tax Year” drop-down box
- Click “Display”
- Click “Printable 1095-C” to generate a printable view of the Form 1095-C
- Print the Form 1095-C using the print feature on your web browser (ctrl + p)
